Looking for a particular CDC guidance document

I am trying to find (unsuccessfully) a CDC guidance document that had a chart and guidelines as to what you do if a person/employee has contact with someone who has tested positive for Covid, has had contact with someone who had contact with someone else who had Covid (i.e twice removed), etc. and so on.  There was a chart out there, it had a nice graphic showing the recommendations for direct contact, somewhat removed contact, etc.

If anyone has any idea what I am talking about please share!  Driving me nuts that I can't now find it.  Happy Friday!
