Hi folks, Our state's Labor & Industries department has issued guidance requiring employers to continue to require employees to wear masks in indoor settings or, alternatively, verify the vaccination status of individuals who wish to not wear a mask. Of course, the guidance is not accompanied by a suggested verification form. Is anyone able to provide a form that might be suitable for this task?
